Our Practice Policies
Appointments and Scheduling
If you need to change a follow-up appointment, please notify our office at least 24 hours in advance. If you need to re-schedule or cancel a procedure, you must notify our office at least 72 hours in advance to avoid a cancellation fee.
You may request renewals on the Patient Portal or have your pharmacy fax a request. If a prior authorization is required, please call our office.

If you are using medicare as a primary or secondary insurance and are 65 or older, please print and fill out this form.
We participate with the following insurance. This is subject to change, please call the office to verify. We no longer accept Medicaid.
